Describe the bug

I just installed Phoenix Code on MacOS Tahoe 26.2. It completely overrode ALL file associations without asking, which has broken many other apps I use daily.

This is terrible UX behavior and effectively a breaking bug.

Reproduction

1. Install Phoenix Code on MacOS
2. All file associations are automatically switched to Phoenix Code e.g. .md, .html etc
3. No permission, no easy undo.

Expected behavior

1. Install Phoenix Code on MacOS
2. Interactive dialog asking permissions for each file type
3. User selects file types to be associated with Phoenix Code
4. Installation completes
5. Entire system of existing apps not broken.
