feat: show loading indicator during SPA navigation fetch

During SPA navigation (link interception + fetch), no loading indicator is shown. The user has no feedback that a link click is loading.

Expected behavior: Show the loading indicator before fetch, hide after response or error.

Discovered during cross-handler SPA navigation research (PR #58).
