Nested repeats don't start with a concrete instance

In https://github.com/XLSForm/pyxform/issues/182, we made it so every repeat has a concrete instance. However, https://github.com/getodk/web-forms/issues/380#issuecomment-3989858514 makes it seem like that doesn't apply correctly to nested repeat.
