Caption of the table

A table caption is in the "to-do" list for an upcoming release. A PR to implement it would be great, if you're in a hurry.

• Centered text (possibly multiple lines)
• Above or (default) below table? Allow two-part caption above and below table?
• Repeat on every page? Does the caption count against the height of the table?
• Within border (with separator rule) or outside of border?
• "(cont.)" set by -cont flag, at top and bottom or just on caption line?

This is all pretty fancy, and a simpler implementation could be acceptable. Should a caption be fixed text, or (like header and footer rows) should it be possible to update its contents? See #52 items 3 and 4.
